Confidentiality

I acknowledge that certain information I will handle, or have access to, during my work as a volunteer with bookmytherapy is confidential. This includes information that relates to clients, participants, staff, placement students/ interns, volunteers and partner agencies.

Code of Ethics

bookmytherapy is committed to the highest level of moral and ethical standards in the delivery of services and the conduct of its work. I agree that I will be respectful, professional when representing bookmytherapy. I will ensure that I protect technology and information assets of bookmytherapy and use them wisely. I will not engage in any criminal activity. I will ensure I speak with my supervisor or the volunteer coordinator immediately if I have any concerns or questions.

Email & Photo Consent

As per the Canadian Anti-Spam Legislation, I am providing my Express Consent to communicate electronically. I understand that my consent may be withdrawn at any time by emailing contact@bookmytherapy.ca. I give bookmytherapy permission to use images of me captured through video, photo or digital camera while I am volunteering with the program, to be used for education and/or promotional purposes. If I do not wish to have my photo used, I will notify bookmytherapy by emailing contact@bookmytherapy.ca

Screening

I authorize bookmytherapy to contact the references I have provided. I agree to provide a police check, should one be required (18+), at my own expense. I certify that all the information included in this application is true and complete and I agree to perform all assigned and accepted responsibilities to the best of my ability.

I authorize bookmytherapy to collect and upload all documents I sign, to a safe and secure volunteer management system.

I also understand that there may not be a volunteer position immediately available at this time and that my information will be kept on file for future openings.
